Hawthorne Health and Rehab of Brandon – Job Function:                       To provide Occupational Therapy Services Supervises:                           No OneHours: PRNReports To:                           Therapy Director  Primary Responsibilities: 1.        Coordinates with Therapy Team to schedule evaluations and Treatment times for potential residents and current caseload.2.        Evaluate and treat Resident’s per Physician Order3.        Responsible for the completion of all documentation related to plan of care, including screenings, evaluations, progress notes, reassessments, and discharge summary.4.        Develop programming consistent with individual resident needs in conjunction with responsible or assigned facility staff.5.        Provide in-service training pertinent to staff education in specialized therapeutic areas, in conjunction with the Therapy Director and other responsible facility staff.6.        Attend facility meetings/in-service trainings as approved by the Therapy Director.7.        Follow Facility Policies and Procedures and State/Federal Guidelines as it pertains to therapy requirements8.        Perform other duties as requested by the Therapy Director. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.   The employee must occasionally lift and / or move / push and / or pull up to 50 lbs.
